Kim Conant been a pediatric nurse for 42 years, and 26 of those years she has spent with Palmetto Pediatrics. She started the unique, (at the time), Special Needs Care Coordination/Medical Home program in September of 2002. Since that time, 16 years ago, she has trained numerous other Care Coordinators. Over her career, Kim has had the opportunity to work with countless multi-disciplinary teams to support families of children with special healthcare needs. Kim feels that an interprofessional practice approach is critical for each special needs child, and their families, to help them reach their ideal outcomes. In her "spare" time, Kim enjoys gardening, reading, scrap-booking, traveling, and spending as mush time as possible with her 3 grandchildren. Unlimited
